Most forms need to be filled out online by logging in to our My GradColl system with your NetID. These forms are designed to be filled out by the student.
These forms require login through My GradColl:

- Master's / Specialist Completion of Degree Requirements
- Doctoral Plan of Study
- Results of the Oral Comprehensive Examination for Doctoral Candidacy
- Committee Appointment (formerly Advancement to Candidacy)
- Announcement of Final Oral Examination
- Completion of degree form for Audiology majors
- Adjustments to Master's/Specialist Plan of Study
- International Special Student Change of Status
- Distribution Rights
- Change of Program
